Faculty wishing to host clinics must seek Chair approval before requesting with the Facilities Office. Once Chair approval has been received, please fill out this form to request space for a clinic.
A few notes about clinics:
- Clinics must be requested at least six weeks in advance. This window of time is needed to ensure University guidelines are followed and these can be adequately staffed and supported.
- Julia Miller will respond to your request within one week once the Production Team has determined whether or not the clinic can be supported with our space and staff resources.
- A tour must be scheduled with the Admissions Office for clinics involving high school students.
- Clinics involving students below high school age must be compliant with the Programs & Events with Minor Children policies. The primary clinician will be responsible for ensuring these requirements are met.
