The Business Office processes all requests and transactions pertaining to budget, contracts, purchasing, payroll, guest payments.
Part-time Faculty Additional Pay Request Form: For payments of studio class teaching and other one-time payments.
Visitor/Guest Payment Request Form: For payments to individuals for masterclasses, lectures, accompanists, sectionals, and other one-time payments. (Department chairs should be consulted in advance of any payment requests).
Form W-9: Required for any non-employee payments made through Accounts Payable for anyone who has (a) not received payment from the university within the last 12 months or (b) has recently changed addresses.
Direct Deposit Enrollment/Change Form: Recommended for any payments made through Accounts Payable. Individuals who do not opt into direct deposit will be paid via a mailed check.
